Houston OCD & Anxiety is dedicated to ensuring you are well aware of your rights to access your records or raise concerns about the care you are receiving. The below information is in accordance with Texas Law (HB 4224):
- To request your Health Records, please email Dr. Ally Sequeira (ally@htxoa.com) or our Program Administrator, Lizzie Sequeira (scheduling@htxoa.com) or send a message through your client portal. Please note that there are some instances in which a release of information form may be required for this release of records. We will inform you when this is needed and guide you through the process.
- Should you have any concerns about your therapist’s licensure, please contact the Texas Behavioral Health Executive Council.
- To file a consumer complaint, please visit the Texas Attorney General’s Consumer Protection Division.
